Overview
Any forms that are not included with the system by default can be uploaded as Custom Forms. These forms can be programmed to populate information dynamically based on the loan file they are generated for. This article will guide you through creating, customizing, and using these forms.
Note: Only PDF files are supported, up to a maximum size of 60MB.
How to Create and Customize a Form
Follow these steps to upload and set up a new custom form.
- Navigate to Company Settings > Forms List.
- Click the Upload button.
- Choose the PDF document from your computer to open it in the form customization screen.
- Click and drag fields from the menu on the left to place them in the document. You can add non-editable text, borrower-fillable fields, dynamic fields that pull from the loan file, checkboxes, and signature blocks.
- Click Save. You will be prompted to name, classify, and choose a location for the new form.

How to Use and Send a Form
Once your form is saved, you can generate it in a loan file and send it for eSignature.
- Navigate to the desired loan file.
- Click Disclosure Forms in the menu on the left, then select Forms.
- Find your new form in the list and click on it.
- Review the populated dynamic fields and click Generate.
- Click Create eSign Need to start the signing process.
- Click Setup e-Sign, enter a message to the borrower, then click Review and Send.
